Our aim and vision for older people is to support Adults to remain at home within their community and have maximum level of independence for as long as possible.  We will ensure there is appropriate level of capacity for Adults with complex needs, and we will work in collaboration with Health and community partners to support people return to their long term home following hospital stays.

  • Refresh of the LAH framework completed, and project established to commence redesign of new contracts to replace current LAH framework
  • Refresh of the IRN Framework completed and project established to replace the IRN Framework.
  • Out to market/tender for Intermediate care contract 
  • Implementation of the initial phase of the carer charter for LAH Providers
  • Overall ambition of the care charter confirmed and implemented. 
  • Enhance the dementia and complex nursing provision approach to shape into future IRN contract or nursing services.
  • (August 2025) New LAH Contract in Place that supports our strategic direction. 
  • (June 2025) New IRN Contract in place that supports our strategic direction, this includes the use of dependency tool for adults with complex needs, 
  • (Sept 2024) New Intermediate Care Contract that supports our strategic direction
  • (2025) New Social Value and Climate baseline and measurements in place (for all new OP contracts) 
  • (August 2025) Baseline and measurement approach for EDI for all new OP contracts 
  • EHM embedded and up and running for LAH and intermediate care 
  • E-Sourcing tool in place to manage the sourcing of our domiciliary market. 
  • Trusted Assessors in place for intermediate care and tier 1 live at home providers 
  • Tech Enabled Care in place for Tier 1 Live at Home Providers 
  • 2025-28 engagement with providers across the care sector and NHS to develop relationships
  • Genuine and established partnership models with Intermediate care services. 
  • Deliver our Climate and Social Value ambitions. 
  • Support Adults to remain at home within their community, with their maximum level of independence sustained for as long as possible
  • Ensure there is the appropriate level of capacity to support adults that require complex support, better understand the scale and nature of increasing complexity of need
  • Increased collaborative discharges for adults entering short term residential or nursing placements from hospital, ensuring we support people to return to their long-term homes as soon as they are able.
  • Reduce the residential placements made in favour of more community-based services such as supported living, shared lives and general needs accommodation
  • Develop in county services for complex provision
  • Continue to investigate and understand demand and to share this with the market
