Urgent Community Response Team (UCRT)

Urgent Community Response Team (UCRT) – The UCRT service supports hospital admission avoidance across Mid and South Essex. Designed with a ‘home first’ ethos, the service will provide specialist, nurse-led care in people’s own homes, it is also available to those in residential care. Care workers can refer in if they are supporting someone that needs some medical support at home, e.g., they have had a minor fall and usually they would reach 999 or an ambulance service – The UCRT may be able to support as an alternative.
