Report an Outbreak tool

The UKHSA have now moved to the use of  the Report an Outbreak Tool (previously known as CareOBRA) which is an electronic web based reporting tool for ALL respiratory outbreak and notification including COVID-19. Therefore, please complete the reporting via the link below and include as much information as you can using the link or the QR code below.

Once a notification is received, the Health Protection Team will risk assess based on the information provided and assess if the information provided via email is sufficient or they need to contact the setting for more information. Therefore, it is very important you complete the form as soon as possible.
