Joint Domestic Homicide and Safeguarding Adults Review for Carol

Please be advised that the report regarding the joint Domestic Homicide Review (DHR) and Safeguarding Adults Review (SAR) for “Carol” has been published on the ESAB website.  A pseudonym has been used to protect the identity of the deceased and her family.

The report was ratified by ESAB on 27 November 2024, and was subsequently submitted to the Home Office QA panel, in line with the procedure for DHRs.  Following their review and feedback, we are now in a position to publish – as you may be aware, there are significant delays in the review of reports by the Home Office panel, which is why we are only now able to publish.

Agencies involved in the review have been notified and where relevant, agencies have been requested to identify actions they have taken or will take, to address the recommendations made within the report. These will be monitored by the SAR Subcommittee on behalf of ESAB.
