Contact information for providers regarding EPUT clients (1)

Please can we request that Providers call the office numbers for care coordinators in the first instance where there are any issues relating to the care provision. Where a Provider cannot locate this information please contact the EPUT switchboard who can direct you to the appropriate team. Tel: 0300 123 0808. The quickest route for any concerns to be addressed is directly through contacting the care coordinator and their team. SPT do not keep up to date information regarding care coordinators for EPUT.

All Providers should regularly update their records with current contact details to prevent any delay in contact and addressing concerns.
